    Types of Assessments 

    Everlasting Wellness offers the following structured adult ADHD assessment services in Pennsylvania that are designed to provide clarity, insight, and support for individuals who may be exploring neurodivergence, pursuing treatment options, accommodations, or understanding their lived experience: 

    • ASRS (Adult ADHD Self-Report Scale v1.1): a standardized screening tool used to assess adult ADHD symptoms.

    • DIVA.2 (Diagnostic Interview for ADHD in Adults): a structured clinical interview that explores symptom history across life stages.

    Who is eligible for Assessments 

    Our assessments are available for individuals 18 years and older in Pennsylvania. At this time, we do not provide assessments for children or adolescents. If you are seeking evaluation for someone under 18, we can help guide you toward appropriate providers.

    Reasons for an Assessment

    People pursue ADHD assessments for a range of personal, clinical, and professional reasons, including:

    • Understanding how ADHD may influence daily life.

    • Supporting informed decisions around treatment and care.

    • Accessing medication or accommodations.

    • Guiding coordination with other healthcare providers.

    Your assessor will explore your motivations and goals at the start of your appointment.

    Understanding & When You May Need a Formal Evaluation

    ADHD assessment processes vary widely. Some institutions (schools, disability services, or prescribers) may require a formal neuropsychological evaluation completed by a licensed psychologist. Our assessments, conducted by a Licensed Clinical Social Worker or Licensed Professional Counselor, are designed to provide structured clinical evaluation but are not a substitute for formal psychological testing. Before your appointment, check requirements from any third-party requesting documentation.

    What to Expect

    1. Pre-Session Preparation: you’ll complete brief consent forms.
    2. Clinical Interview: administration of ASRS and DIVA.2 to explore symptoms, history, and impact.
      This can be scheduled virtual and may take up to 2 hours to complete together.
    3. Interpretation: your assessor reviews findings and determines if ADHD criteria is met.
    4. Recommendations: you’ll receive a summary of results you can use for personal planning, provider discussions, or accommodations. 
    5. Transparent Cost: $400 (self-pay).

    Why not insurance? 

    Insurance coverage for assessments can vary based on factors such as age, the reason for seeking diagnosis, and plan-specific requirements, and some assessments may not be reimbursed at all. Choosing to complete an assessment on a self-pay basis can offer greater privacy, as findings remain between the individual and the assessor rather than being shared with an insurance company or other third parties. Self-pay assessments also allow the assessor to provide a more flexible, non-pathologizing space to thoughtfully explore neurodivergence without the constraints or diagnostic pressures often imposed by insurance guidelines.
